For ease of check in, kindly print & fill out the PATIENT REGISTRATION FORMS and bring them to your appointment
or
arrive 15 minutes early & we will be happy to provide you with paperwork to fill out prior to your exam.
We also ask that you bring your
current glasses, previous glasses prescription, & contact lens information
(physical box or photo of brand & contact lens prescription) to your visit.
We have also provided you with our NOTICE OF PRIVACY PRACTICE for your records.
